Plan Changes Effective January 1, 2008
Chiropractic Benefit is now a $1,000 benefit maximum per calendar year.
Physical Therapy benefit is increased to 15 visits per calendar year.
Occupational Therapy benefit is now limited to $2,500 maximum per
calendar year.
Speech Therapy benefit is now limited to $2,500 maximum per calendar
year.
Mental Health Outpatient benefit is increased to 30 visits per calendar
year
Substance Abuse Outpatient benefit is increased to 30 visits per
calendar year
HPV Vaccine is now allowed under the Routine Wellness Benefit
Hearing Aid benefit is increased to $1,500 every 5 years.
Dental is $100 per individual with a cap of $300 per family per calendar
year.
Short Term Disability (non-occupational) is now a flat rate of $400 per
week, up to 13 weeks.
